Diabetes Canada 2023 Guidelines
Gestational Diabetes Screening & Target Evaluator
Interpret 50g Glucose Challenge Test (GCT) and 75g diagnostic OGTT results, capillary blood glucose targets, and clinical/partner management plans.
50g Glucose Challenge Test (GCT) Input
1-Hour Plasma Glucosemmol/L
US equivalent: ~151 mg/dL
Presets:
Diabetes Canada Thresholds:
• < 7.8 mmol/L: Normal
• 7.8 – 11.0 mmol/L: Proceed to 75g 2-hour fasting OGTT
• ≥ 11.1 mmol/L: Gestational Diabetes confirmed
Canadian Target Blood Sugars in Pregnancy
Fasting / Pre-Meal< 5.3 mmol/L (< 95 mg/dL)
1-Hour Post-Meal< 7.8 mmol/L (< 140 mg/dL)
2-Hour Post-Meal< 6.7 mmol/L (< 120 mg/dL)
Target HbA1c< 6.5% (ideally < 6.0%)
Evaluation ResultOGTT Required
Borderline Screen: Confirmatory 75g OGTT Required (7.8 – 11.0 mmol/L)
A 1-hour glucose of 8.4 mmol/L falls in the borderline zone (7.8 to 11.0 mmol/L). A diagnostic 75g 2-hour fasting Oral Glucose Tolerance Test (OGTT) must be scheduled.
Diabetes Canada / SOGC Clinical Directives
- •Order formal 75g 2-hour fasting OGTT (e.g., Dynacare / Manitoba lab).
- •Instruct patient on preparation: Eat a standard, unrestricted carbohydrate diet (≥ 150g carbs/day) for 3 days prior; fast for 8–12 hours overnight before test.
- •Patient must remain seated and refrain from smoking during the 2-hour test interval.
- •Diagnosis will be established if ANY ONE of the three plasma glucose values (fasting, 1h, 2h) meets or exceeds cutoff.
High-Yield Partner Daily Habits for GDM
Post-Meal Strolls: A brisk 15-min walk immediately after supper drops peak 1-hour blood glucose by 1.0 to 1.5 mmol/L.
Prep the Bedtime Snack: Pair whole carbohydrates with healthy protein/fat (Greek yogurt + handful of walnuts) to prevent nighttime Dawn phenomenon spikes.
Newborn Birth Plan: Plan for immediate uninterrupted skin-to-skin contact for the first hour to prevent neonatal hypoglycemia.
